    Summary

    Housing and Urban Development/VA Supportive Housing (HUD-VASH) is an intensive case management program for chronically homeless Veterans aimed at providing psychosocial services to Veterans while residing in stable housing through HUD's Section 8 voucher system. The incumbent in this position functions as the Case Manager for West Texas VA Health Care System's HUD-VASH Program at the Permian Basin Community Based Outpatient Clinic located in Odessa, Texas.

    Responsibilities

    Individuals assigned as GS-9 grade level social workers are considered to be at the entry level and are closely supervised, as they are not yet functioning at the independent practice level conferred by independent licensure or certification. Social workers at the GS-9 entry level are typically assigned to program areas that do not require specialized knowledge or experience. The GS-11 is the full performance level where Social workers are licensed or certified to independently practice social work.

    Functions of the position include but are not limited to:
    *Provides case management for a caseload of Veterans in the HUD-VASH program in the assigned area of responsibility.
    *Reviews agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements.
    *Performs site visits of HUD-VASH apartments to ensure that Veterans reside in safe environments in compliance with local housing codes.
    *Verifies the Veteran's status, eligibility for VA Health Care System treatment, and eligibility of program participation.
    *Ensures homeless Veterans are considered as primary participants for program acceptance.
    *Collects and submits HUD-VASH Program participant data as outlined by NEPEC evaluation procedures.
    *Provides HUD-VASH case management. Case management is defined as the provision of services by VA clinical staff to homeless Veterans and includes all activities to provide appropriate treatment and maintain Veterans in HUD approved stable housing.
    *Coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery, and linking and referring Veterans to the Community Based Outpatient Centers, VA West Texas VA Health Care System, VA Regional Offices and/or community agencies.
    *Acts as an advocate or authorized representative in accordance to SSI/SSDI Outreach, Access, and Recovery (SOAR) in accordance and completion of the training by Substance Abuse and mental Health Services Administration (SAMHSA).
    *Complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of interest while serving as the HUD-VASH Case Manager.
    *Actively participates in the HUD-VASH Program conference calls and attends training offered by the HUD-VASH.
    *Follows local policy and SOP for the HUD-VASH Program. This includes following policies and documentation requirements as required for CARF and TJC accreditation.
    *Actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences.
    *Takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested.
    *Performs accountability functions: 1) document all contacts with patients/families in the medical charts; 2) completes statistical and other reports in accordance with West Texas VA Health Care System and respective professional discipline service policies.
    *Works with West Texas VA Health Care System multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to HUD-VASH Program Veterans.

    The incumbent is assigned to the Social Work Service and reports to the HCHV Homeless Social Work Supervisor. The HCHV Homeless Social Work Supervisor provides programmatic oversight to the HUD-VASH Program. The incumbent provides comprehensive social work services to Veterans and their families in the assigned area of responsibility. Responsibilities include the management, coordination and provision of social work services to Veteran and their families in inpatient, outpatient and community settings. In the provision of care in patients from the adult (less than 65 years of age) to the geriatric (greater than 65 years of age), the employee recognizes the age related needs and demonstrates knowledge and care appropriate to that age group. In the performance of official duties, the incumbent has regular access to the printed and electronic files containing sensitive data which must be protected under provision of the Privacy Act of 1974 and other applicable laws, Federal Regulations, VA statues and policy. Incumbent is responsible for protecting data from unauthorized release of access codes, etc., as stated in the computer access agreement which the incumbent signs.

    Work Schedule: Monday - Friday 8:00 a.m. to 4:30 p.m.
