    The Clinical Pharmacy Specialist is located at the VA Central California Health Care System for Acute Care settings and special procedures such as Operating Room, PACU, GI Lab, Ambulatory Infusion/Oncology, Nuclear Medicine, and Emergency Department.

    The Clinical Pharmacy Specialist (CPS) at VA Central California Health Care System functions at the highest level of clinical practice and works under a scope of practice as approved by the PSS/Medical Staff Executive Committee. The CPS functions to initiate, modify, discontinue, and monitor medication therapy and acts as a consultant for medication therapy management services. This includes, but is not limited to: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.

    Work Schedule: Monday Through Friday, 8 AM to 4:30 PM, Variable - Rotations on weekends and evenings.
